- You can contact us online, by phone or in person.
- To begin, you’ll need to complete a triage form—this can be done on our website, over the phone, or at the reception desk.
- A doctor will review your form to help us prioritise urgent needs and offer the most suitable care.
- Depending on your needs, appointments may be offered by phone, video, or face-to-face.
You can complete the form Monday to Friday, between 8:00 AM and 6:30 PM
Cancelling or changing an appointment
To cancel your appointment:
- Use your NHS account (through the NHS website or NHS App)
- phone us on 01889 562145, Monday to Friday from 8am to 6.30pm
- reply CANCEL to your appointment reminder text message
If you need help when we are closed
If you need medical help now, use NHS 111 online or call 111.
NHS 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.
Call 999 in a medical or mental health emergency. This is when someone is seriously ill or injured and their life is at risk.
If you need help with your appointment
Please tell us:
- if there’s a doctor, nurse or other health professional you would prefer to respond
- if you would prefer to consult with the doctor or nurse by phone, face-to-face, by video call or by text or email
- if you need an interpreter
- if you have any other access or communication needs
Home visits
If you are housebound and need an appointment, we will do a home visit. We will phone you first to understand what you need.
If a home visit is needed, the request should be made before 12:00 noon by telephoning the surgery on 01889 562145.
Non-urgent advice: Notice
We are able to provide a much better quality of care at the surgery than in your own home. Doctors can see approximately five patients in the surgery in the time it takes to visit one patient at home. It is for these reasons that we would ask all patients to attend the surgery whenever possible.
General Non-Urgent Appointments with our nursing team
You can book an appointment with a member of our nursing team online, by phone, or at our reception desk.
There’s no need to complete a triage form for these appointments.
When booking, our reception team will ask for the reason for your appointment to ensure you are booked into the most appropriate slot.
If you choose to book online, please refer to the guide below to help you select the correct appointment type.
If an appointment is booked incorrectly, you may receive a text message cancelling the appointment. Where possible, we will include a link to help you rebook into the correct slot.

Extended Access
Additonal appointments are offered across the whole of East Staffordshire, including evening and weekend appointments.
